YMCA Community As Medicine Learning Collaborative

Our 2024 "Community as Medicine Learning Collaborative" brings together 4 Y's from across the country to build equity-centered group health coaching in partnership with local clinical healthcare.

Recipe4Health 

We partner with Recipe4Health's "Food as Medicine" program, amplifying the impact of produce prescriptions by providing comprehensive behavioral support, nutrition education, and coaching in partnership with local FQHCs.

A Few of our Clinical & Housing Partners

Currently, OSW receives and processes an average of 400 patient referrals per month. Our coaches conduct text and phone-based outreach, build relationships with them, and (based on their individual interests, needs, and challenges) enroll them into our virtual or in-person services, including clinic-based Group Medical Visits, community-based group coaching, and individual coaching.
